About the Author
Lloyd R. Goodwin, Jr., Ph.D., LPC, CRC-MAC is interim chairman of the Department of Rehabilitation Studies. He is also director and professor in the Substance Abuse and Clinical Counseling program within the Department of Rehabilitation Studies, School of Allied Health Sciences at East Carolina University in Greenville, North Carolina. He teaches graduate courses in clinical, substance abuse, and rehabilitation counseling. He has maintained a full or part-time private consulting and clinical counseling practice off and on since 1968. Dr. Goodwin initiated and helped develop the first national and North Carolina master's degree level certification in substance abuse counseling. Dr. Goodwin earned his Ph.D. in Counseling, Health and Rehabilitation, with a major in Rehabilitation Counseling from Florida State University in 1974. He has been on the faculty of the State University of New York at Buffalo, Assumption College, and Emporia State University prior to his current position at East Carolina University where he has been since 1989. He has worked in the counseling field for 33 years as either a clinical counselor or counselor educator.
Dr. Goodwin is a Licensed Professional Counselor (LPC) and Certified Rehabilitation Counselor (CRC) with a specialty certification in addictions counseling (Masters Addictions Counselor; MAC). Dr. Goodwin is founder and a past-president of the national Professional Association of Rehabilitation Counselors (PARC).

Button Therapy includes a six-step psychological self-help method based on a holistic cognitive therapy model.
Answer the following questions truthfully, as the CSAI is primarily an inventory to explore and self-assess your thoughts and other cognitions. You are fooling nobody but yourself by not providing honest responses.
Compare Your Responses to a Sample of Adults. When you are finished completing the CSAI you may want to compare your responses to a sample of 293 adults who have taken the CSAI as part of a norming group. The results from this norming group follow the questions on the CSAI. Most subjects (81percent) in this sample were undergraduate or graduate college students ranging in age from 19 to 63 years of age and their responses may not be representative of your cognitions.
Professional Counseling Aid. There are many advantages to bringing your completed CSAI to a mental health professional. A professional counselor can help you explore your responses, insights, and questions. Oftentimes, you may not receive accurate unbiased feedback from a spouse or close friend. A counselor can help you explore your cognitive, as well as emotional, behavioral, and interpersonal domains. A counselor can give you unbiased feedback to help you identify and explore thought distortions, self-defeating core beliefs, defense mechanisms, personality traits, and Buttons that may be causing negative feelings and hindering your personal growth and self-actualization.
Your professional counselor may ask you to complete the CSAI at home and return it for use in your counseling sessions. Your answers on the CSAI are confidential and will be used by your mental health professional as part of your therapeutic counseling.
The CSAI takes approximately 60 minutes to complete.
